Transitional care is critical for reducing hospital readmissions and improving patient outcomes, but it requires careful evaluation of eligibility criteria and the creation of actionable care plans. This agent aims to streamline this process by automating the review of discharge medical records and aligning them with TCM policies.

The agent operates by first retrieving the relevant TCM policy from a designated UiPath Context Grounding index. It then reviews the provided discharge medical record input to determine if the patient meets the eligibility criteria outlined in the policy. If eligibility is established, the agent justifies its decision with references to specific policy rules and creates a detailed transitional care action plan tailored to the patient's needs. This ensures that the process is both compliant and efficient.

The Transitional Care Management Agent leverages UiPath Context Grounding which provides access to TCM policy documents and follows a structured set of instructions to evaluate eligibility and generate action plans. The output includes a clear eligibility assessment, a justification for the decision, and a proposed action plan, ensuring transparency and actionable insights for healthcare providers.

How to use

  1. Click the Download button.
  2. In the Downloading an agent template window, select Download Solution. A .uis file containing the agent definition is downloaded to your machine.
  3. Agent templates can also include Extras, a separate package containing assets for your agent (automations, custom escalations). Select Download Extras. A .zip file is downloaded to your machine.
  4. In Studio Web, import the Agent definition .uis file as described in Importing a project downloaded from Studio Web.
  5. Next, go to Solutions. For details, refer to Accessing the Solutions UI.
  6. Upload the .zip file, as explained in Transporting a solution package, then deploy the solution. For details, refer to Deploying the solution.
  7. You can now access your agent template from Studio Web and add any other tools deployed to your tenant.
